Boarding Pre-Check In with NO Exam
Boarding Information - Please read:
Please be aware that all boarding dogs will be walked three times a day Mondays through Saturdays and twice a day on Sundays.
In case of illness or injury, I, the undersigned, do hereby give my authorization and consent for the staff of East Marietta Animal Hospital to treat, prescribe for, or perform surgery on my pet(s) while they are being boarded at the hospital. I also understand that every effort will be made by East Marietta Animal Hospital to contact me by phone prior to performing any services that are necessary due to illness or injury.
East Marietta Animal Hospital, its doctors and staff will use all responsible precautions against illness, injury, or escape of any boarded or hospitalized pet(s), but they will not be held liable or responsible in any manner whatsoever, under any circumstances, on account of the care, treatment, or safe keeping of my pet(s), as it is thoroughly understood I assume all risks.
Should the circumstances arise that my pet(s) remain unclaimed after the scheduled pick up date, I understand that written notice will be mailed to the address listed on the pet(s) medical record notifying me of such case. Fourteen days after such written notice, the pet(s) will be considered abandoned and may be taken to the Cobb County Animal Shelter. It is further understood that such action will not relieve me of any costs incurred while boarding and any and all costs incurred by East Marietta Animal Hospital as result of pet(s) abandonment.
I hereby declare that my pet is current on all immunizations required for boarding and that my pet(s) are free of internal and external parasites. I also agree that the required immunizations have been given within the last 12 months. If I cannot show written proof (medical record from previous veterinarian with name, address and phone number of veterinarian) of such immunizations, I hereby give East Marietta Animal Hospital permission to administer any and all required immunizations and check for or treat for internal and/or external parasites at my expense.
We ask that boarders limit belongings to food and treats. These items should be able to fit into a plastic bin that is 23” long by 16” wide by 6” deep.
Due to additional time involved for administering medication to patients, pets requiring more than 2 medications daily will have a higher charge than pets needing 0-2 medications.

- Diabetic pets needing insulin injections can have those administered for $10.00 per injection.
- If your diabetic pet is not eating, glucose checks costing $16.50 each will be necessary to ensure your pet can have insulin administered.
